Wiltshire Hospital Discharge Service are looking to recruit an Experienced Social Worker to join their busy team in West Wiltshire. The team is a small and enthusiastic team that work together to achieve safe and timely transfers of care and support patients already discharged from hospital to Discharge to Assess beds in the community.

Your new role
You will hold a caseload and undertake complex assessments to facilitate discharges from both the acute hospital setting and D2A, as well as undertaking the Investigating Officer role in safeguarding cases. Knowledge of the Mental Capacity Act 2005 and the Care Act 2014 is essential and you will have experience in applying these legislative frameworks, as well as supporting others to apply the principles of the Act(s). As a team, they are keen to work on the basis of ‘Home First’ always striving to support the customer to return to their own home on discharge wherever possible.

What you’ll need to succeed
You will have 2 years post qualification experience in a social care setting and have extensive professional development since qualifying and be qualified to a DipSW level or equivalent and registered as a social worker with Social Work England.
