This part time post of community care worker will work predominantly with the Housing Occupational Therapist for owner occupied and housing association properties. However, if service needs change , the post holder may also be requested to work as part of the wider team carrying out assessments for equipment and adaptations.
The role of the community care worker in Housing adaptations is to monitor and progress straight forward cases, follow-up equipment, progress installation of grab rails and support the client where appropriate during major adaptation process. At times additional roles may be required to support the person for example organising care or respite.
The post holder should have skills, experience and knowledge of Occupational Therapy and housing adaptations and working with the third sector.
Within defined boundaries, co-ordinate the provision of a range of services and equipment to meet the assessed needs of individuals requiring assistance, to enable them to maintain and maximise their independence, prevent admission to hospital or a care home, facilitate hospital discharge and to make choices.

Relevant Qualifications
Educated to HNC or SQA Level 3 in social care or equivalent and be able to demonstrate equivalent knowledge, skills and competencies gained through relevant experience.
Driving Licence
As the duties of this role require you to effectively travel to successfully undertake the full remit of the role, you must hold a current driving licence and have access to a vehicle.*

This post is considered as Regulated Work with vulnerable children and/or protected adults, as specified in the Protection of Vulnerable Groups (Scotland) Act 2007.
