An exciting opportunity has arisen for a Community Care Co-ordinator to join either the Adult Assessment or Older People Services Team. As part of the West Lothian Health and Social Care Partnership (HSCP), the team’s provide an assessment, care management and review service to adults and older people.
West Lothian Council is committed to supporting unpaid carers of adults affected by disability. A key role for the successful applicant will be identifying and providing the necessary support to unpaid carers. They will be responsible for working with unpaid carers to develop their Adult Care and Support Plan.
The post holder will be required to think creativity around how we support unpaid carers, whilst assessing for eligibility and formalised respite/support, whilst also offering a sound knowledge of what is available within communities and how this can be accessed to provide support and meet support needs.
Successful applicants must have the ability to work as part of a team, respond to the demands of a busy service, be able to follow procedures, have excellent communication and good report writing skills. They must have the ability to travel throughout West Lothian and hold a recognised qualification in social care.

This position requires registration with the SSSC. The successful candidate will have six months from commencement in this post to register with the SSSC. If you are already registered with SSSC you will be required to add West Lothian Council to your account
This post is considered Regulated Work with Vulnerable Children and/or Protected Adults, under the Protection of Vulnerable Groups (Scotland) Act 2007. Preferred candidates will be required to join the PVG Scheme or undergo a PVG Scheme update check prior to a formal offer of employment being made by West Lothian Council.
