An opportunity within the Home Assessment Recovery Team (HART) is available for two full time Community Care Assistants. You will require strong assessments skills and be able to to work in collaboration and partnership with other teams and social care professionals to carry out comprehensive assessments and reviews of adult clients. You must be able to work autonomously and within a team and demonstrate a commitment to improving service provision. You will ensure appropriate services and pathways are planned and arranged in a timely and effective manner. Due to the nature of the work, a driving licence is essential.
HART is based on Early Intervention and Prevention, promoting a reablement model, aiming for people to reach maximum potential. We support Adults age 16+ across all geographic areas of Perth and Kinross. This work is supported by a range of partners including GP’s and third sector organisations. Our aim is to enable people to live more independently in their communities – providing care and support at the right time, in the right place, when it is most needed.
The successful applicants will assist in supporting people to remain at home by providing a range of social care and care management tasks. These include carrying out Outcome Focussed Assessments, the negotiation of flexible packages of care, and the monitoring and reviewing of individual care packages delivered via SDS (Self Directed Support). You will have the opportunity to participate in on the job training and your continuous development will be encouraged and supported. Benefits also include flexible working scheme. We are looking for an enthusiastic, committed and motivated individual with a can do attitude. You must have excellent communication skills both verbal and written. Experience of managing a caseload autonomously is essential. The ability to be flexible and respond to crisis/ changing needs of individuals is integral to this role. You must have strong assessment skills, and be able to demonstrate an understanding of SDS and how this is delivered. You must also be able to demonstrate a sound knowledge of Social Work Values and Principles. You will be part of the Health and Social Care Partnership and ideally you will have experience of working in a multi-disciplinary setting. Knowledge of the local resources within communities would be an advantage. Knowledge of Adult Support and Protection and Adults with Incapacity legislation, along with an understanding of Carers legislation would be desirable.
