I am recruiting for a Care Coordinator to join an established team in Liverpool. We support and care for individuals who have a requirement for care including personal care within their own homes. Our company values are dedication, enthusiasm and passion.

The role will requirements include:

• Answer telephone enquiries
• Processing referrals
• Schedule clients appointments
• Achieve monthly KPI’s
• Carry out risk assessments and assisting with care plans
• Keeping records of clients up to date
• Work with third parties
• Ensure compliance is met within CQC guidance
• 29 Days Holiday per year
• Negotiable and competitive salary
• Bonus available
