Adult Services Co-ordinator (Temporary)
The social work team based at the Royal Alexandra Hospital is looking to appoint 2 x Adult Service Co-ordinator for a period of 24 months, as part of their new patient flow and discharge arrangements.
As a member of Renfrewshire’s Health and Care Partnership, you will be responsible for identifying patient’s social care needs, organising immediate supports and coordinating arrangements for patients to be discharged from hospital. This will include providing information about care services and coordinating arrangements for care homes, care at home services, housing related support, transport and meals deliveries.
You will be supported by RHSCP colleagues working across locality services, care at home services, care home services and community health. You will develop strong working relationships with acute hospital staff to plan discharges and increase knowledge and awareness of available community supports.
The team is located at the Royal Alexandra Hospital and operate in accordance with the Council’s WORKSMART arrangements which promote mobile and flexible work practices. All Social Workers are considered to be and equipped as mobile workers and can operate throughout the Renfrewshire area from any Council premises. You will therefore be expected to travel throughout the Council area.
Effective interpersonal, communication, IT and sound assessment and writing skills are essential. You must also be able to prioritise your work load, work as part of a team, use your initiative and be innovative and practical. You must be flexible to meet the needs of the service and service users, be motivated and be committed to social work values. You must be able to travel throughout the Renfrewshire area.

This position is covered by The Protection of Vulnerable Groups (Scotland) Act 2007 (‘the PVG Act’) and has been designated as carrying out regulated work with a child or protected adult under the terms of the Act. Consequently, individuals who are named on the Disqualified from Working with Children/Adult Lists will commit an offence if they apply for this post and may face prosecution.
