This is a key post that resources reablement plans, tackles social isolation, is a gentle introduction for older people to services, and enables those living with frailty to continue to enjoy living a safe and fulfilled life in their community for as long as they are able. It requires some flexibility, a can do attitude, and an ability to work autonomously and is an effective team member.
