Report: Logan Mwangi Child Practice Review – The Cwm Taf Safeguarding Board

There is an obligation for all agencies involved in the care, support and protection of children, young people and adults at risk to ensure that the highest possible standards of care, support and protection are provided and maintained at all times. Part of this obligation is a requirement to learn from mistakes, especially those resulting in death or serious injury.

In accordance with the Social Services and Well Being (Wales) Act 2014, the Cwm Taf Safeguarding Board have a responsibility to establish a Practice Review when the criteria requiring one are met.

A Practice Review is held if;

  • abuse or neglect is known or suspected and the individual has died;
  • or sustained potentially life threatening injury;
  • or sustained serious and permanent impairment of health or development
