PURPOSE OF JOB: To provide a seven day service providing alternatives to hospital admission and reducing length of hospital stay. The postholder will deliver programmes of care, support and rehabilitation to clients in their own homes. The aim is to assist clients to maintain or regain independence, support them to maintain their physical, mental and social well being and to work with them to address risks which may lead to loss of independence. MAJOR TASKS: To act as Key Worker contributing to the assessment of individuals, carrying out a range of practical tasks and delivering planned programmes of occupational therapy and physiotherapy rehabilitation. 60% To travel between team base and clients’ homes and with clients in cars (full driving licence required) and on public transport as part of a programme of rehabilitation. 25% To report and share appropriate information regarding the client in a timely and accurate manner. 15%
JOB ACTIVITIES: To undertake, record and share an initial assessment of the support needs of people at the first point of contact during a period of crisis To undertake an initial health and safety risk assessment in clients’ own homes. As part of the team, to participate in formulating and implementing plans to support clients in their own homes to avoid hospital admission. To assist individuals to achieve their goals by delivering rehabilitation interventions relating to activities of daily living, exercise programmes and planned activity programmes, including the delivery of aspects of the falls pathway To liaise and co-operate with informal carers and other members of the multidisciplinary team in relation to clients’ changing needs. To work as part of the team to organise own and others’ daily activity to ensure that clients receive visits in accordance with their care and/or treatment plan.
To assist clients to achieve community access in accordance with their treatment plan, assisting with mobility and using public transport and cars as required To order, fit, demonstrate and promote appropriate use of equipment in accordance with agreed competencies and local procedures. To verbally report updates in relation to client’s needs to other members of the multidisciplinary team and progress interventions or actions as appropriate. To record accurate, timely, concise and reliable written reports, using electronic information management systems as required. To contribute to team meetings and attend multi –agency discussions as appropriate. To carry out ward visits as required to familiarise clients with the service and to establish the client’s expectations. To provide shadowing opportunities for new colleagues or students on placement To collate and input data as part of the monitoring and evaluation of the service To participate in activities which promote learning and development and contribute to service quality. To support the team with any delegated office administration tasks as required.
Knowledge and Skills Essential The postholder must possess or be willing to undertake HNC/SVQ level 3 in H & SC or equivalent level relevant qualification. Full current Driving Licence Desirable Knowledge of the national care standards Awareness of personalisation and outcomes agenda Good communication and interpersonal skills Literacy skills Skilled in time management and team working Involvement in assessment and care planning Basic computer skills/typing Experience Experience of working with people receiving services from Health or Social Care services Experience of working independently and as part of a team Experience of caring for individuals with a range of complex care needs Ability to report accurately verbally and in writing
This post is regulated work with children and/or protected adults under the Protection of Vulnerable Groups (Scotland) Act 2007. The preferred candidate will be required to join the PVG Scheme or undergo a PVG Scheme update check. Where an individual has spent a continuous period of 3 months or more out with the UK in the last 5 years, an Overseas Criminal Record Check will be required. You will be required to provide this check. An unconditional offer of employment and commencement in the post will be subject to the outcome of both these pre-employment checks being deemed satisfactory.

Our salary range typically reflects the initial starting salary and annually increases until it reaches the top of the range.
